Consent & Acknowledgment

I acknowledge that participation in the Future Ready Through Pickleball program ("the Program") involves physical activity, including but not limited to pickleball play, movement drills, and related on-court exercises. I understand that these activities carry inherent risks, including the risk of injury, and that the participant is participating voluntarily. I, as parent or legal guardian of the named minor participant, agree to assume all risks associated with the participant's involvement in the Program. On behalf of myself, the participant, and our heirs, I release and hold harmless Charlotte Financial Pickleball Club ("CFPC"), its operators, facilitators, coaches, venue partners, volunteers, and agents from any and all claims, demands, or causes of action arising out of or related to the participant's participation in the Program, except to the extent caused by the gross negligence or willful misconduct of CFPC. I acknowledge that this waiver is being signed by me on behalf of the minor participant named below, and that the minor is identified in this agreement.

In the event of an injury or medical emergency during Program activities, I authorize CFPC, its facilitators, and venue partners to secure necessary emergency medical treatment for the named minor participant. I understand that CFPC will make reasonable efforts to contact me (or the emergency contact I provide) before treatment, but if I cannot be reached, I authorize treatment deemed necessary by a licensed medical professional. I confirm that the participant does not have any medical condition that would make participation unsafe unless previously disclosed in writing to CFPC. I am responsible for informing CFPC in writing of any allergies, conditions, or medications relevant to the participant's safety.

I grant CFPC permission to photograph, film, or otherwise record the named minor participant during Program activities, and to use such images, video, or recordings in CFPC marketing materials, community content, website, and social media. I understand that the participant's full name will not be published alongside images without separate written permission. I may withdraw this consent at any time by notifying CFPC in writing; withdrawal does not affect use that occurred before notice was received. I acknowledge that I am the parent or legal guardian authorized to grant this permission on behalf of the minor.
